Overview
Intuitive Surgical, Inc. (ISRG) develops technology for minimally invasive care, principally the da Vinci surgical platform and Ion endoluminal system. It earns revenue from instruments and accessories consumed in procedures, system sales and leases, and services. In Q2 FY2026, those categories produced $1.73 billion, $685 million and $472 million of revenue, respectively, according to the Intuitive Surgical Q2 FY2026 earnings release. The company operates globally and sells or leases systems to healthcare providers, but geographic revenue concentration was not disclosed in the supplied materials. Its competitive environment includes alternative surgical approaches and other robotic-assisted systems, while adoption depends partly on hospital capital budgets, clinical acceptance, regulatory clearances and the availability of trained care teams.

What changed this quarter
Intuitive Surgical’s second-quarter change was not simply another period of double-digit procedure growth. Revenue increased 19% year over year to $2.89 billion, ahead of the $2.44 billion reported in Q2 FY2025. Combined da Vinci and Ion procedures grew approximately 16%; da Vinci procedures rose about 15%, while Ion procedures increased approximately 36%. Revenue therefore grew faster than aggregate procedure volume, helped by higher da Vinci leasing revenue and expansion of the installed base, according to the Intuitive Surgical Q2 FY2026 earnings release.
The composition was constructive. Instruments and accessories revenue rose 18% to $1.73 billion, preserving the link between utilization and recurring consumables. Systems revenue increased 19% to $685 million, reflecting a larger lease base, higher average selling prices and more placements. Services revenue reached $472 million, up from $391 million one year earlier. Intuitive placed 468 da Vinci systems, versus 395, including 246 da Vinci 5 systems compared with 180 in the prior-year period.
The installed base continued to widen. Da Vinci systems reached 11,710 at June 30, 2026, up 12% year over year, while the Ion installed base increased 21% to 1,096. More placements were made under operating leases: 254 da Vinci systems, including 131 usage-based leases, compared with 193 and 124, respectively, a year earlier. That shift can defer upfront revenue but can also align Intuitive more closely with customer utilization over time.
The largest financial change was margin conversion. GAAP gross margin improved to 67.8% from 66.3%, and GAAP operating income rose 31% to $972 million. Operating margin reached 33.6%, 3.1 percentage points above the prior-year quarter and the highest level in the supplied 12-quarter table. Free cash flow increased to $1.06 billion from $0.72 billion. These figures are consistent with the income statement and cash-flow data in the Intuitive Surgical Q2 FY2026 Form 10-Q.
Not all of the improvement was recurring. GAAP EPS rose to $2.29 from $1.81, but the quarter included a $28 million after-tax benefit, or $0.08 per diluted share, from refunds of tariffs paid in prior periods. Excluding only that item, EPS would have been $2.21. Share-based compensation also remained substantial at $213 million, up from $200 million, and should not be treated as a temporary expense.
Why it matters for the thesis
The quarter strengthens the operating side of the thesis in two ways. First, procedure growth remains broad enough to support the consumables model. Da Vinci procedures grew 15%, Ion procedures grew 36%, and instruments and accessories revenue advanced 18%. That combination indicates that the installed base is not merely expanding; it is being used. Each additional active system can contribute instruments, accessories and service revenue after the initial placement.
Second, revenue is growing faster than procedures without requiring margin sacrifice. Higher system placements, pricing and leasing revenue helped total revenue outpace combined procedure growth, while operating expenses rose much more slowly than gross profit. GAAP gross profit increased to $1.96 billion from $1.62 billion, while operating expenses rose to $989 million from $875 million. That operating leverage is the quarter’s most important evidence because Intuitive’s valuation requires growth to translate into cash earnings rather than being absorbed by commercial and development spending.
Management did not materially raise the full-year framework. It continues to expect worldwide da Vinci procedure growth of 13.5% to 15.5%, with results closer to the midpoint. The company also maintained non-GAAP gross-margin guidance of 68.0% to 69.0% and non-GAAP operating-expense growth of 11% to 13%. Q2 da Vinci procedure growth of approximately 15% sits within that annual range, so the print supports the outlook rather than establishing a new growth trajectory.
The balance sheet limits financing risk. Intuitive ended June with $8.63 billion of cash, cash equivalents and investments and $2.58 billion of total liabilities. It generated enough cash to repurchase 0.9 million shares for $380 million while increasing cash and investments by $650 million during the quarter. The Intuitive Surgical Q2 FY2026 Form 10-Q also reported $18.30 billion of stockholders’ equity at quarter-end.
The tension is valuation. At $360.46, Intuitive trades at 41.3x trailing EPS and offers a 2.9% trailing free-cash-flow yield. Those figures are compatible with a durable, cash-generative growth company, but they already presume that procedure expansion and operating leverage persist. The quarter reduced concern about near-term execution; it did not create a wide valuation discount.
What INTUITIVE SURGICAL INC is worth after the print
There is no prior SageNoodle fair value to carry forward, so this update initiates a $366 base-case value. The method applies scenario P/E multiples to the supplied trailing EPS of $8.72. This is deliberately transparent: the valuation does not silently forecast undisclosed consensus earnings, and the scenario multiples express the return investors may require under different procedure-growth and margin outcomes.
The bear case applies 32x trailing EPS for a value of approximately $279. That assumes procedure growth falls to 13.5% or below, operating margin returns toward 29%, and tariffs or hospital spending pressure weaken earnings conversion. The multiple would still recognize Intuitive’s installed base and balance-sheet strength, but it would remove part of the premium attached to sustained mid-teens growth.
The base case applies 42x trailing EPS, producing approximately $366 per share. It assumes procedure growth near the 14.5% midpoint of management’s range, continued installed-base expansion and an operating margin around 31% through a normal mix of quarters. The multiple is close to the current 41.3x trailing P/E because Q2 confirmed, rather than fundamentally changed, the company’s growth and quality profile.
The bull case applies 52x trailing EPS for a value of approximately $453. It assumes procedure growth reaches 15.5% or better, da Vinci 5 and Ion adoption sustain above-base growth, and operating margin holds near 33% as recurring revenue scales. This outcome requires both superior execution and continued willingness by the market to pay a high multiple.
At $360.46, the $366 base value implies only 1.5% upside and therefore a Fairly Valued verdict under SageNoodle’s rules. The strong quarter supports the existing market premium, but a 2.9% free-cash-flow yield offers little protection if procedures slow or margins normalize. EV/EBITDA is not disclosed in the supplied materials and is therefore not used in the valuation.
What could prove this wrong
The clearest risk is that procedure growth decelerates below management’s 13.5% to 15.5% range. Instruments and accessories are the largest revenue category, so weaker utilization would affect both growth and the economics of usage-based leases. A larger installed base is valuable only if system use remains healthy.
Margins could also prove less durable than Q2 suggests. The quarter benefited from prior-period tariff refunds, and management’s non-GAAP gross-margin guidance already includes an estimated tariff cost equal to 1.0% of revenue. The company warned that additional tariffs could have a material effect. Intuitive manufactures a significant majority of instruments and accessories in Mexico, a majority of endoscopes in Germany, and imports certain materials from China, creating multiple points of trade exposure.
Hospital capital spending and leasing economics are another uncertainty. More than half of Q2 da Vinci placements were operating leases. Leasing can expand access and recurring revenue, but it increases Intuitive’s capital commitment and makes returns more dependent on future utilization. Delayed hospital spending, staffing constraints or weaker procedure demand could lengthen the payback period.
Competition, regulation and supply concentration remain structural risks. The company identifies competing robotic systems, including domestic competitors in China, as well as possible regulatory delays, manufacturing disruption and reliance on sole- and single-source suppliers. Any development that slows placements or causes customers to question safety, training or clinical value would challenge both the growth assumption and the premium multiple.
Finally, the valuation itself can invalidate an otherwise sound operating thesis. At 41.3x trailing earnings, Intuitive does not need a business failure to underperform; a lower multiple caused by slower growth or higher required returns would be enough. The thesis would need revision if procedure growth falls below the guided range, GAAP operating margin settles materially below 29%, or recurring revenue ceases to track installed-base growth.
